Responsibilities Pathways Center Care Campus has two 24/7 Crisis Stabilization Units; one for adults and one for individuals ages 5-17. Pathways Center's Crisis Stabilization Units (CSU's) are short term (2-7 days), medically monitored crisis residential programs, which offer services for a limited duration to stabilize acute psychiatric or behavioral symptoms, evaluate treatment needs, and develop plans to meet the needs of persons served. The programs operate 24 hours per day 7 days per week. The CSU's are designed to serve as a first line of community based alternative care to hospitalization, offering psychiatric stabilization and detoxification services on a short term basis. In addition to the crisis stabilization programs, Pathways Center Care Campus has a Crisis Service Center (CSC). The CSC provides on-site and telephonic crisis intervention and 23-hour observation services 24 hours a day, 7 days per week. The CSC operates on a walk-in basis and provides psychiatric/substance abuse related crisis evaluation and brief intervention services to support an individual who is experiencing an abrupt and substantial change in behavior noted by severe impairment of functioning typically associated with a precipitating situation or a marked increase in personal distress. Interventions are provided by licensed behavioral health professionals designed to prevent out of community treatment or hospitalization. Based out of the Care Campus is the Coweta CARES emergency response program. Coweta CARES is a unique and developing para-medicine program that is designed to address behavioral health crisis calls to 911. Coweta CARES is a team of licensed clinicians and paramedics who work collaboratively to stabilize individuals who are experiencing behavioral health crises. The goal is to reduce unnecessary hospitalization and ED presentations. The Licensed Counselor for the Care Campus rotates throughout all programs to ensure timely and quality care therefore the schedule will vary. The Licensed Counselor role has performance targets/productivity expectations to ensure quality care as well as compliance with regulatory and governing entities. Responsibilities for the Licensed Counselor will include but not be limited to the following: Provides patients/clients with the psychosocial and therapeutic support needed to cope with behavioral health crisis and/or detoxification Completes Behavioral Health Assessments (BHA's) as a part of the intake process within the required timeframes. Conducts clinical interviews with the residents and is able to make a diagnostic impression based on clinical assessment Works with other healthcare team members to prepare for and facilitate individual and group discussions on the unit with patients/clients to promote and reinforce the recovery process Provides individual therapy and may conduct family sessions as needed to help facilitate a successful transition back to the community Develops treatment plans collaboratively with the patient/client and input from all treatment team members Provides professional level therapeutic services to persons with mental illness and/or addictive disease Conducts face-to-face comprehensive assessments to identify problems and service needs Works alongside EMS personnel in the Coweta Cares program Provides risk assessment and crisis intervention Facilitates the admission process and maintains communication with referral sources, families, patients and Care Campus healthcare team Must be proficient in use of technology with respect to use of Electronic Medical Record. Must be able to work on a rotating basis within the Care Campus and Coweta Cares; providing coverage for all programs |
Minimum Qualifications LPC, LCSW, or LMFT license in Georgia A minimum of 1 year experience working in a behavioral health setting. |
Preferred Qualifications Additional certification as a Substance Abuse Counselor (CAC, CACI, CACII, etc) is preferred |
